Whether you want to work on your own, with a group or one-on-one with a social worker – or any combination of the three – North Durham Family Health Team is here for you. We can also help you navigate your options and adjust your care. We offer a variety of resources and levels of support to meet you where you are, and to help you move forward on the path that’s right for you. All clinicians are registered with the Ontario College of Social Workers and therapists hold a Master of Social Work.
Please note mental health programming through the North Durham Family Health Team is intended for clients with mild to moderate mental health concerns. If you are feeling distressed, learn more about resources here to support you.

Any of these can be a first step in connecting to available resources and helping you find the care you need, when you need it. You can use these services in a combination that is right for you, and as your mental health changes, your supports can change with it.
There’s no one right way to address your mental health—any or all of these options may be helpful to you. North Durham Family Health Team is committed to providing easy access to the services, supports and resources you need.

Self-Guided Support
There are lots of ways to access mental health information, education, and support, as well as opportunities to learn independently.

Group Support
Group therapies can help you connect with others while learning new skills and coping strategies in a safe, supportive space. Cognitive behavioural therapy can enhance your coping skills and find the help you need to succeed.
Group therapies are a great way to try evidence-based tools and techniques, or to supplement the care you’re already receiving. All group therapies have scheduled dates and times. Options include cognitive behavioural therapy for depression and anxiety, as well as, cognitive behavioural therapy for insomnia.

Individual Counselling
Meet with a social worker for brief individual therapy (8-10 sessions). One-on-one therapy appointments are reserved for those who are clients of Medical Associates of Port Perry and nurse practitioners of the North Durham Family Health Team, and do not have access to health care benefits or an Employee Assistance Program through their employer. Therapists hold a Master of Social Work.

Single Session Clinic
If you feel like you need mental health support or are unsure what type of care you might need, you can meet with a social worker to discuss concerns, learn about resources or receive a single session of solution-focused therapy. Concerns that you may bring to the clinic could include accessing services/support in the community, feeling sad, anxious or worried, relationship concerns, concerns about children, situational stressors.
One session is often enough to address your needs, but you can book subsequent appointments to build on previous sessions or to address something new.

Workshops
Group education can provide insight and information on a variety of topics related to health and wellness. All group workshops have scheduled dates and times.

Community Referrals
If you need support outside of what the North Durham Family Health Team offers, we can help connect you to resources in the community, including peer support, social connections, counselling/psychotherapy or other services.
